Welcome to Ascension Preschool Academy!

Thank You for choosing our preschool and for entrusting your child to our care. We are looking forward to the beginning of a long lasting partnership in education with you and your child!

The forms you will need to complete the enrollment process for your child and information about immunizations required to be distributed by us are listed below. The parent handbook has a lot of important information, and we know that you will review it all carefully.
All forms must be signed by parents or guardians.

Due as soon as possible to guarantee your child’s placement:
•Payment of last month’s tuition (May) and registration fee. The deadline is due one week prior to enrollment if you have not paid.
•Regular monthly tuition is due by the 5th of each month.
•Enrollment contract
•Acknowledgement of Receiving and Reviewing Parent Handbook
•Emergency Information
•Sunscreen Form
•Child Information Form
•Child’s statement of Health Form
•Immunization Print-out from your Physician or completed by his/her office
•Permission for use of photos
If your child requires daily medication or needs to keep emergency medication on the premises, please let us know as soon as possible and we will provide you with the necessary Permission for Medication form or Health Care Plan as required.

On your child’s first day please bring back (labeled with name)
•2 pictures of your child (for emergency cards)
•A complete change of clothes (pants, shirt, sweater (if winter), socks, underwear)
